What are the responsibilities and job description for the Care Coordinator for Non-Profit position at PathWays of the River Valley?
At PathWays of the River Valley, we have been providing quality services to individuals with developmental disabilities and their families for over 50 years. Our region consists of Sullivan and Lower Grafton County in New Hampshire, and our programs work with individuals from birth throughout their lifespan as needed.
This full-time position is based in Claremont, NH, with regional travel and/or within-state travel required. The starting pay range is $21.00 to $23.00 per hour depending on experience.
As a Family Services Coordinator (FSC), you will play a crucial role in assisting individuals with developmental disabilities and their families by providing service coordination and advocacy while linking them to existing community and state resources.
Your responsibilities will include:
- Advocating and supporting families and individuals in a manner that increases opportunities for participation in the community
- Coordinating assessments and evaluations for all individuals as assigned
- Securing necessary referrals, documentation, and relevant data to determine individual needs
- Facilitating planning meetings for individuals and completing related documents as requested or required
This position requires a Bachelor's degree, extensive Case Management experience, or a combination of both. You must also have at least 2 years of human services experience with experience working with developmental disabilities highly desired.
You will be responsible for:
- Providing excellent verbal, written, and interpersonal communication skills
- Holding a minimum of intermediate proficiency with MS Office (Word, Excel, Outlook)
- Being willing and able to travel regionally or within the state of NH
- Having access to reliable transportation
This position reports directly to the Director of Family Services. All PathWays positions offer a HIRING BONUS, and this position requires passing a criminal background check and other requirements listed below.
