What are the responsibilities and job description for the Family Support Coordinator for Non-Profit position at PathWays of the River Valley?
For over 50 years, PathWays has been providing quality services to individuals with developmental disabilities and their families. Our region consists of Sullivan and Lower Grafton County in NH and our programs work with individuals from birth and throughout their lifespan as needed. This position works out of Claremont, NH with regional travel and / or within-state travel and hybrid work may be available after your first 90 days of employment. In addition, the position is full-time and the starting pay range is $21.00 to $23.00 per hour depending on experience.
Reports to the Director of Family Support
Do you like assisting others in accessing necessary resources and broadening their involvement in the community? In this role, you assist individuals with developmental disabilities and their families by providing service coordination and advocacy while linking them to existing community and state resources. This position also assists individuals with budget administration. You balance the use of informal and formal resources while maximizing family control over services. At the request and direction of the family / individual, you serve as a liaison with community, state agencies, and service providers. You understand and promote the transition process and support individuals / families through planning the transition from the educational system to adult supports / services, including natural supports and the provider selection process. You facilitate the legal process for individuals / families as necessary including Guardianship, Power of Attorney, Advance Directive planning, and Representative payee. Knowledge of benefits will be a key component of this position, and you will build a strong knowledge base of adult services and transitions from school services to adult services. You will identify and connect families to local and statewide resources and facilitate planning meetings, ISAs, amendments, and action plans. The ideal candidate will offer flexibility, be a creative problem solver and collaborative team player, and have a strong background with documentation. Experience working with families from diverse backgrounds in a non-judgmental manner is necessary and knowledge of IEPs is a plus. Home visits are required, which necessitates reliable transportation. We will train the right person.
This position requires :
- Bachelor's degree is highly preferred; extensive case management experience will be considered in place of degree
- A minimum of 2 years of human services experience is necessary with at least 1 year in Family Support; experience with developmental disabilities is preferred
- Excellent verbal, written, and interpersonal communication skills are a must
- Willingness and ability to travel either regionally or within the state of NH are a must. Access to reliable transportation is required.
- Minimum of intermediate proficiency with MS Office (Word, Excel, Outlook) is required and the ability / willingness to learn new programs is a must.
- Knowledge of Medicaid and SSI benefits is preferred while strong organizational skills are necessary
