Job Summary
The Case Manager is responsible for assessing, planning, implementing, monitoring and advocating for the children in our residential programs in support of their permanency goals. Responsibilities include establishing relationships with the children in order to identify their individual needs as well as maintaining documentation for how their needs are being met and monitored on a regular, on-going basis. The Case Manager will work with other Case Managers, community resources and other community-based providers/representatives. The Case Manager arranges, coordinates, monitors, evaluates and advocates for appropriate services to meet a childs specific needs.
Essential Duties:
Program:
- Ensures compliance with all regulatory, licensing, and accreditation requirements.
- Support the Program Administrator and Treatment Director with program implementation.
- Makes recommendations to Program Administrator for program improvements.
- Ensure all childrens records, including medical and educational files are current with appropriate documentation included, such as admission assessments, diagnostic assessment and treatment plans, therapy notes, plans of service/reviews; medical, dental, psychiatric and psychological exams and reports; current court orders and appropriate consents and agreements; report cards, educational testing, discipline referrals, etc. Prepares and submits documentation as required for utilization review by Youth for Tomorrow.
- Coordinates and arranges transportation and staffing for planned outings as needed.
- Participates in staffing meetings as member of the interdisciplinary team as needed.
- Works with Program Administrator in providing community awareness and support for the program.
- Develops professional relationships with other peers in the field.
Clients:
- Responds to referral requests and reviews and screens admissions applications timely. Coordinates the admissions process and may schedule pre-admission visits.
- Provides input on appropriateness of placement for client referrals.
- Develop and facilitate implementation of admission assessments, diagnostic assessment and service plans to ensure quality care and compliance with all regulatory, licensing, accreditation, and Youth for Tomorrow Program Standards.
- Completes monthly documentation on how the child is progressing.
- Monitors other staffs documentation on the implementation of each childs Individual Service Plan.
- Provides accurate information to staff of each childs special needs for community outings and activities (i.e., allergies, diet restrictions, sun sensitivity, significant likes and dislikes, etc.).
- Ensures needs of all children in care are being met including purchase of clothing, supplies and gifts.
- Arranges for the transport of children to healthcare and other appointments.
- Coordinates with psychiatrist, pharmacy and Medicaid as necessary to ensure medications are on hand per doctors orders; and that the medications are administered correctly to the child, as needed.
- Ensures maintenance of daily log for each child and maintains all client documentation in the electronic record.
- Effectively and efficiently collaborate with and communicate information regarding children on his/her caseload to team members including lead staff, direct support professionals, therapist, teachers, psychiatrists, parents/guardians, attorney ad litem, etc.
- Adopts a proactive stance on behalf of children on his/her caseload. Ensure all children served have appropriate advocacy support, internally and externally.
- Demonstrate proficiency in childrens psychological disorders, particularly those common in the abused/neglected child.
- Refers to community agencies as necessary and appropriate.
- Acts as a resource to staff on habilitation and behavior management.
- Identifies and seeks resolution for potential problems.
- Participates in required annual training necessary for program standards to include management of aggressive behavior and psychotropic medications as required by the program.
- Attends ARDs (Admission, Review and Dismissal), parent/teacher conferences, court hearings, Texas Department of Family and Protective Services staffing etc.
Supervisor: Program Administrator.

Work Environment: Office and residential setting interacting with clients. May be exposed to blood or bodily fluids.
Work Hours: This is a full-time, minimum 40 hours per week position. Work hours must be flexible to include some weekend, holiday, and evening hours.
Classification: Non-exempt.
Education & Experience
- High school diploma or equivalent. Bachelors Degree in a behavioral science or related field preferred.
- A minimum of two years of case management experience in a therapeutic program serving children in a residential treatment or other child welfare setting.
- Experience working with children diagnosed with behavioral and emotional disorders and intellectual and developmental disabilities is strongly preferred.
Knowledge, Skills, & Abilities
- Experience with foster and/or adoptive children and families and knowledge of the TDFPS system is preferred.
- Knowledge of child growth and development concepts and ability to understand and relate well to children.
- Strong knowledge of behavioral and emotional disorders and intellectual and development disabilities depending on the program assigned.
- Ability to support the agency's culture, growth, and success through communication, accountability, and positivity.
- Ability to be clear headed and decisive based on the scope of the position.
- Ability to work efficiently and effectively both individually and as part of a team.
- Ability to appropriately accept feedback through the supervision process thus displaying the willingness to learn, grow, and improve.
- Competent in using Microsoft Word, Excel, the Internet, and other software applications.
- Effective oral/written communication and organizational skills.
- Ability to effectively work as a part of a professional team.
Additional Requirements
- Proof of valid Texas Driver's License (Type C) and at least three years of driving experience.
- Access to reliable transportation.

- Cleared criminal background check and signed statement regarding felony indictments/convictions.
- Cleared TB test results (current within 12 months prior to employment).

Physical Demands

Manual dexterity for computer use. Occasional reaching with hands and arms. Additional physical requirements may include, frequent lifting and or moving up to 25 pounds.
