JOB SUMMARY:

Clinical Pharmacists are licensed pharmacists who are responsible for providing safe and appropriate pharmacy services to patients in accordance with the policies and procedures of the company, the state’s professional standards/regulations, and other regulatory entities. Clinical Pharmacists are responsible for reviewing and approving all prescriptions dispensed, assessing patients from a clinical status, and evaluating the appropriateness and effectiveness of provided therapies/medications.  Clinical Pharmacists are responsible for ensuring that all processes fall within the scope of pharmacy practice. 

 


QUALIFICATIONS:

  • Graduate of an accredited school of pharmacy, licensed and in good standing with the Florida State Board of Pharmacy. 
  • Advanced knowledge of English, both verbal and written, including correct grammatical form is required. 
  • Advanced knowledge of pharmacy principles and practices.   Execution of “Best Practices” techniques in day-to-day work activities is essential.
  • Excellent written and verbal communication skills. 
  • Flexibility and ability to work as part of a multi-disciplinary team.
  • Self-directed with excellent time management skills and able to work independently, occasionally under stressful conditions.  
  • Minimum of two years experience in a hospital, home infusion or long-term care pharmacy setting. 
  • Exposure to a fast-paced, deadline-driven environment. 
  • Ability to effectively prioritize, adapt and multitask, excellent organizational skills.
  • Ability to effectively supervise pharmacy staff. 
  • Ability to prepare and analyze reports.
  • Ability to identify problems within the work routine and can source and partner with management staff to correct and facilitate work processes
  • Advanced computer skills including use of standard office applications including Microsoft Office (PowerPoint, Excel, etc.)
  • Ability to learn and master industry-specific software applications (i.e., CPR ) 
  • Excellent customer service skills.
  • Knowledge in nutrition support therapies preferred.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Oversee operations of the pharmacy department to effectively manage the dispensing and shipping processes according to all regulatory requirements.
  • Direct staff productivity to ensure timely completion of duties within established policies and in accordance with Board of Pharmacy requirements.
  • Follow established procedures for drug and supply procurement and assist in maintaining appropriate inventory levels and preferred product selection of drugs,
  • Provide exceptional service to patients, and internal and external customers and interact professionally with all contacts.
  • Review patient prescriptions and make suitable recommendations for modifications in the prescribed medication therapy and/or monitoring parameters for patients.
  • Attend/complete educational programs or participate in other CEU opportunities to promote professional competency and comply with state-mandated CEU requirements.
  • Provide concise, applicable, and timely responses to requests for drug information from clinics, health care providers, and patients.
  • Formulate responses to drug information requests based on analysis of the literature.
  • Provide medication consultation services as requested by patients.
  • Obtain, interpret, and evaluate all physician’s medication orders to assure therapeutic appropriateness, cost-effectiveness, accuracy, and completeness. 
  • Assist in receiving initial prescription orders and secure patient demographic information.
  • Perform accurate computer entry, labeling, and dispensing in accordance with Board of Pharmacy regulations. 
  • Perform data entry to maintain patient profiles, including a list of medications taken by individual patients.  
  • Accurately maintain all legally required records. 
  • Perform clinical reviews of patient documents such as history and physical, physician and dietitian notes, and monthly lab reports to assess medical necessity.
  • Review formulations, labs, medications, clinical notes/patient information, etc., and update patient care plans as per pharmacy policy.
  • Perform patient medication reviews to monitor for drug-drug or drug-therapy interactions.
  • Provide technical and clinical information and communicate results of monitoring activities to patients, nurses, and physicians as required regarding the contents, utilization, therapeutic actions, possible adverse reactions, or interactions of drugs.
  • Ensure the accuracy of the work of Pharmacy Technicians and Support personnel.
  • Supervise IV compounding activities/all technicians while actively compounding in the IV room, checking in the workroom, and the pharmacy admin area.
  • Assist in training and orientation of Pharmacy Technicians, Support Staff, and Registered Dietitians. 
  • Successfully complete Media Fill and Gloved Fingertip testing after initial training and on annual basis.
  • Successfully complete the Aseptic Technique and yearly didactic exams. 
  • Adhere to stability and compatibility guidelines and quality control procedures.
  • Assist in the shipping, receiving, and delivery of products as necessary.
  • Work with the Pharmacy Operations Manager to manage the process for assessing and reporting medication-related incidents (QREs) and equipment errors and/or failures to the company QA/PI committee. 
  • Participate in Performance Improvement / Quality Assurance activities, educational programs, and departmental meetings.
  • Identify ways to improve and promote quality processes within and outside of the department.
  • Work cohesively with and delegate appropriately to other team members including Patient Care Coordination and Billing departments, registered dietitians, pharmacy technicians, and pharmacy support staff. 
  • Communicate effectively with teammates and other departments to provide information/data as needed and assist with appeals/denials and insurance re-authorizations.
  • Support the team culture within the organization by adhering to policies, practices, and the Company’s mission statement.
  • Keep abreast of new pharmacy technologies and practices.
  • Perform job responsibilities within established regulatory guidelines (HIPAA/ PHI) and reports non-complain activity to the Compliance department; protect patient and company confidentiality in all matters.
  • Maintain knowledge of all regulatory requirements by attending in-service classes and annual HIPAA updates.
  • Ensure compliance with federal, state, and local laws and regulations, policies and procedures, and accrediting body standards relating to the practice of pharmacy.
  • Be on time and ready to work; adhere to the company Dress Code by maintaining a professional appearance in appropriate attire.
  • Other duties as directed by business conditions or management directives.
