Role Description
This is a full-time hybrid role as a Service Coordinator at Patient Care Coordination, Inc. The Service Coordinator will be responsible for day-to-day service coordination tasks to connect participants with community services. This role is based in Harrisburg, PA, and the surrounding counties with the flexibility for some work from home. Caseload for the position would range between 50-60 participants.

- Service Coordination and Care Management skills
- Strong Interpersonal Skills and Communication abilities
- Customer Service experience
- Ability to work independently and in a team
- Detail-oriented and organized approach to tasks
- Experience in the healthcare or social services field is a plus
- Bachelor's degree in Social Work, Human Services, or related field
SALARY RANGE: $37460.00 - $48960.00 Annually
