Essential Functions :
NPs in skilled nursing facilities and / or acting in Cardiac Program lead position will participate in facility roundsNPs in skilled nursing facilities , providing Gap care visits, acting as, and / or treating patient in home who PCMG is not the Primary Care or attending physician evaluate and discuss with staff physicians and PCPs as necessaryReview any abnormal vitals and labs and make recommendations and / or notify the attending physician.Facility providers Review assigned new admissions and their orders.Participate in readmission review meetings and provide a summary of all PCMG acute discharges using the acute discharge worksheet and will review PCMG patients weekly with IDT (interdisciplinary team)Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patientsOrders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needsPrescribes appropriate pharmacologic and non-pharmacologic treatment modalitiesImplements interventions to support the patient to regain or maintain physiologic stability, including but not limited to serving in the first assisting role or working with PCPMonitors the effectiveness of interventionsFacilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patientsCollaborates with multidisciplinary team members by making appropriate referralsFacilitates staff, patient and family decision making by providing educational toolsExperience and Education :
NP Degree from a College or University and two years related experience and / or training.Physical Requirements :
