About Us :
Bedrock / Patient Care Therapy Group (PCTG), is a house-call primary care therapy practice where rehabilitation associates and clinical operators collaborate to maximize the functional mobility of our patients. Unlike the traditional route of reactive care, our team focuses on strategically educating and proactively addressing fall prevention and conditions that may delay the optimization of our patient outcomes. PCTG applies a functional medicine approach to all care plans and designs preventative & rehabilitative solutions to combat all movement, cognitive, physical, and language disorders. By combining holistic clinical practice with functional medicine in the most practical environment—the comfort of their own home—our patients can age in place and truly enhance their quality of life. Patient Care Therapy Group is a clinician-led practice that prioritizes quality patient care and therapist well-being. We value our therapists and empower them with the autonomy to manage their caseloads, provide individualized patient-centered care, and to develop their professional skills.
