Job Summary
The Registered Nurse (RN) in Home Health provides nursing care to patients in their homes as prescribed by physicians and adheres to applicable laws, regulations, and agency policies.
- You will coordinate patient care plans with other healthcare professionals and ensure continuity of care through effective communication.
- Makes initial and comprehensive nursing evaluations, develops plans of care within state guidelines, and documents accurately and timely in collaboration with physician orders.
- Ensures patients meet home health eligibility and medical necessity requirements.
