We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health Nurse Coordinator. In this role, you will be responsible for providing comprehensive nursing care to patients in their homes, working collaboratively with healthcare professionals to develop and implement individualized care plans.
About the Role:
- Initiate, develop, implement, and make necessary revisions to patient care plans in collaboration with physicians and other healthcare professionals.
- Directly supervise care provided by home health aides and licensed practical vocational nurses, provide instruction as needed, and assign tasks according to state and federal regulations.
- Ensure accurate and timely completion of comprehensive assessments (OASIS), including medication reconciliation.
Responsibilities:
- Provide high-quality clinical services within scope of practice, adhering to infection control standards and in accordance with patient care plans.
- Communicate effectively with patients, families, caregivers, and other healthcare professionals to ensure continuity of care.
- Accurately document patient visits per policy and payer requirements, ensuring timely submission of documentation.
Requirements:
- Current RN licensure in state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to reliable transportation
