Responsibilities
- BUILD TRUSTED NURSE-PATIENT RELATIONSHIPS
- Develop, implement, and revise care plans in collaboration with physicians and healthcare professionals.
- Directly or indirectly supervise home health aides and licensed practical vocational nurses, providing instruction as needed and assigning tasks according to state and federal regulations.
Clinical Services
- Provide high-quality clinical services within scope of practice and infection control standards, adhering to the plan of care and coordinating with other healthcare team members.
- Accurately document patient visits per policy and payer requirements, submitting timely documentation as per LHC policy.
Quality Initiatives
- Conduct initial and comprehensive nursing evaluation visits, ensuring patients meet home health eligibility and medical necessity guidelines.
- Communicate relevant information effectively with agency staff, including patient care issues, visit assignments, and schedule changes.
Professional Expectations
- Arrive at work with a positive attitude, willing to help patients and the agency in any way possible.
- Take direction from Clinical Director and Executive Director, completing tasks assigned in a timely manner and assisting with the orientation of new personnel.
Requirements
- Current RN licensure in state of practice
- Schedule flexibility to ensure patient care needs are met
