Job Summary
Patient Centered Care is a Nurse Practitioner owned primary care practice opened since 2007 in Wilmington & 2017 in Bolivia. We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. This role requires a strong clinical background, excellent communication skills, and a commitment to delivering high-quality patient care in a collaborative environment. Providers do not see more then 12 scheduled patients a day. This position is for someone who is looking for a slower care environment.
Responsibilities
- Conduct thorough patient assessments, including physical examinations and health histories.
- Develop and implement individualized care plans based on patient needs and evidence-based practices.
- Being a team player; to not only be focused on provider duties but be an extra hand if the MA or Nurse needs assistance with lab draws, vaccines, EKGs, answering the phones to name a few.
- Provide education and counseling to patients and their families regarding health management and disease prevention.
- Manage chronic conditions through regular follow-ups and adjustments to treatment plans as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Maintain accurate medical records in compliance with legal and ethical standards.
- Participate in clinical research initiatives as needed to enhance patient care practices.
- Engage in infection control measures, including sterile processing techniques and airway management when required.
Qualifications
- Master’s degree in Nursing with a focus on Family Nurse Practitioner studies.
- Current certification as a Family Nurse Practitioner from an accredited organization.
- Valid state nursing license with the ability to practice independently.
- Must have at least 1 year experience as a provider. Prefer 2 years and Primary care experience.
- Commitment to ongoing professional development and adherence to best practices in healthcare delivery.

Office Hours (current)
- Monday- Thursday 7:30-4pm
- Some Fridays
- No weekends
- Schedule rotates between both locations; Wilmington & Bolivia
