We are looking to bring on a Social Worker (BSW or LSW) as a Care Coordinator that will work with the Patient Sortal Health medical team to coordinate care for Patient Sortal members. MUST be licensed in New Jersey AND / OR Pennsylvania.

The individual in this position will function as part of an interdisciplinary team, supporting Patient Sortal Health and is responsible for managing Patient Sortal members through their clinical care coordination program. He or she will work along with the team to connect participants with community and social services, ultimately with the aim of improving patient care outcomes and quality of life and reducing unnecessary healthcare utilization. This is a virtual position for the most part with room for face to face capacities when necessary or desired.
Requirements:
- Experience in care coordination, social services and/or behavioral.
- 3 years in healthcare setting.
- Excellent communication skills.
- Previous experience working with Returning Citizens.
Responsibilities:
- Provides virtual (mostly), in-person (bi-weekly), telephonic, and written communication support for Patient Sortal members.
- Addresses participant social, functional and continuing care needs in a timely and effective manner.
- Screens patients/clients for services and intervention appropriately as evidenced by appropriate documentation and services.
- Documents communication and collaborates with Patient Sortal Health's team to communicate and implement plan of care to meet participant needs.
- Evaluates data and information to accurately assess need for community and social services.
- Establishes and maintains updated community and social services resources and ensures continuity with services as identified across the healthcare system and Patient Sortal members.
- Demonstrates knowledge and empathy regarding participants care needs, as well as impact of associated social determinants of health, which may include financial, psychosocial and knowledge barriers.
- Actively participates in the realization of participant care goals and documents all interaction with the participant, associated caregivers, and resources.
- Communicates consistently and in positive manner with Patient Sortal staff, members, and associated providers across the healthcare system and community.
- Respects and incorporates direction provided by the Patient Sortal Health lead.
- Provides weekly reporting of participant interaction and progress toward attainment of care goal, via case review with Nurse Practitioner(s).
- Updates Patient Sortal team as directed by supervisor and is accountable for participant outcomes.
- Monitors and evaluates participant response to planning, services and intervention in order to assure quality of services and make improvement where necessary.
- Weekly participant follow up through direct contact, via face-to-face or phone, in order to evaluate progress towards short and long term goals.
- Actively participates in goal refinement and continually strives to optimize participant and caregiver engagement.
Credentials - Essential:
- RN - Registered Nurse with Care Management experience
or
- BSW - Bachelors in Social Work
- LSW - Licensed Social Worker
Competencies and Skills - Essential:
- Clear Communication Skills Both Written And Verbal
- Knowledge Of Basic Computer Programs
- Able To Keep Confidential Information Regarding Patients, Team Members
- Able To Withstand Crisis Situations
