For over a decade, we’ve been leading the way in home-based primary and palliative care—and we’re still growing! We’re looking for a dedicated Medical Doctor (MD / DO) to start part-time with the opportunity to grow into a full-time role.

Duties to include but not limited to :

  • Conduct in-home visits to assess, diagnose, and manage acute and chronic medical conditions.
  • Develop and implement personalized care plans for homebound patients.
  • Work closely with an interdisciplinary care team (NPs, PAs, nurses, social workers, and case managers) to coordinate care.
  • Manage polypharmacy, preventive care, and chronic disease management for patients with complex medical conditions.
  • Utilize electronic medical records (EMR) for accurate documentation and compliance with regulatory standards with notes being locked in 72hours.
  • Provide end-of-life care discussions, palliative care planning, and advance care directives as needed.
  • Educate patients and caregivers on disease management, medication adherence, and home safety .
  • Participate in on-call rotations (varies by practice)
  • Makes appropriate referrals to home health or hospice as needed.
  • Facilitates access to appropriate supportive care services .
  • Serves and protects the health care provider practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
  • Updates job knowledge by participating in educational opportunities ; reading professional publications; maintaining personal networks; participating in professional organizations.
  • Seeing on average 5-6 patients per day in homes and 8-12 patients per day in facilities.

  • Requirements :

    1. Education & Licensure

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school
  • Completion of residency in Family Medicine, Internal Medicine, or Geriatrics (preferred)
  • Board certification in a relevant specialty (strongly preferred, sometimes required)
  • Active medical license in the state of practice
  • DEA registration for prescribing controlled substances
  • Medicare and Medicaid enrollment
  • 2. Experience

  • 1-3 years of experience in primary care, geriatrics, palliative care, or internal medicine (preferred)
  • Experience in home-based primary care, house calls, hospice, or palliative care is a plus
  • Comfort with managing complex, chronic conditions in frail, homebound, or elderly patients
  • Experience working in interdisciplinary teams (e.g., collaborating with nurse practitioners, physician assistants, nurses, and social workers)
  • 3. Skills & Competencies

  • Strong clinical judgment and ability to work independently in a home setting
  • Ability to perform in-home assessments and manage acute and chronic conditions
  • Familiarity with electronic medical records (EMR) and documentation standards
  • Knowledge of home health, hospice, and palliative care services
  • Strong communication and interpersonal skills for patient and family education
  • Ability to manage polypharmacy and coordinate care with specialists
  • 4. Other Requirements

  • Valid driver’s license and reliable transportation for home visits
  • Comfort with traveling within a designated service area
  • Ability to lift and carry medical equipment as needed
  • Willingness to participate in on-call rotation
