Duties to include but not limited to :
- Conduct in-home visits to assess, diagnose, and manage acute and chronic medical conditions.
- Develop and implement personalized care plans for homebound patients.
- Work closely with an interdisciplinary care team (NPs, PAs, nurses, social workers, and case managers) to coordinate care.
- Manage polypharmacy, preventive care, and chronic disease management for patients with complex medical conditions.
- Utilize electronic medical records (EMR) for accurate documentation and compliance with regulatory standards with notes being locked in 72hours.
- Provide end-of-life care discussions, palliative care planning, and advance care directives as needed.
- Educate patients and caregivers on disease management, medication adherence, and home safety .
- Participate in on-call rotations (varies by practice)
- Makes appropriate referrals to home health or hospice as needed.
- Facilitates access to appropriate supportive care services .
- Serves and protects the health care provider practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
- Updates job knowledge by participating in educational opportunities ; reading professional publications; maintaining personal networks; participating in professional organizations.
- Seeing on average 5-6 patients per day in homes and 8-12 patients per day in facilities.
