Duties
- Conduct comprehensive patient assessments to develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patient progress, maintaining accurate medical records.
- Provide education and support to patients and their families regarding health management.
- Collaborate with interdisciplinary teams to ensure coordinated care.
- Assist with activities of daily living (ADLs) as needed.
- Respond promptly to patient needs and changes in condition.
- Ensure compliance with safety protocols and infection control measures.
Skills
- Strong background in patient care, particularly in home health settings.
- Experience in critical care environments such as Level I Trauma Centers or inpatient facilities.
- Proficient in managing medical records and documentation practices.
- Ability to work independently while demonstrating excellent decision-making skills.
- Effective communication skills for interacting with patients, families, and healthcare teams.
- Knowledge of clinic operations and the ability to provide compassionate care in a home environment.
