This position is paid at a competitive per-visit rateMileage reimbursement for any company travelCompany 401K with 4% company matchingAs a Registered Nurse Case Manager, you will plan, coordinate, and provide hospice care in accordance with the patient's plan of care.
As a PRN Nurse, you will :
Coordinate hospice care by working with all team members as well as patients and family members.Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.Perform evaluations for hospice care along with informational visits as needed.Submit complete and accurate clinical notes in a timely manner.Participate in IDG and case conferences.Perform on-call duties (including holidays) per policy and agency need.All other duties as assigned.Qualifications :
Must be a graduate from an accredited School of NursingMust be licensed in the State as a Registered Nurse or Licensed Practical NurseOne or more years of experience in hospice preferredMust have knowledge of Medicare and Medicaid guidelinesMust have a criminal background checkMust have a current CPR (BLS) certificationJob Conditions :
Must have a driver's license and be willing and able to drive to patient's residencesThe ability to access patients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient carePhysical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patientsThis position is paid at a competitive PRN rateWe cannot wait for you to join our team!
