Responsibilities
What You Will Do:
- Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
- Provide personal care to our patients, including dressing, mobility, administering medication, personal hygiene, eating and transporting to doctor’s appointments
- Communicate with family regarding patient’s condition
- Document and timely report any changes in the patient’s health status
- Timely complete records in accordance with Company policy
Qualifications
Qualifications:
- High school diploma
- Valid drivers’ license
- CNA state certification (required)
- Prior experience as a CNA (required)
