Position Summary

The primary purpose of your job position is to provide spiritual care and support to terminally ill patients, their families, and caregivers, either directly or through coordination of care with other spiritual/religious counselors. You will serve as part of the interdisciplinary team (IDT). As a Hospice Chaplain, you are delegated the administrative authority, responsibility, and accountability necessary to carry out your assigned duties. You may also be required to provide emotional support to hospice staff through individual counseling or structured group meetings as needed or as directed.

Duties and Responsibilities

  • Stewards the philosophy/mission of PH Healthcare, Inc. which focuses on the overriding values of human dignity and respectful responses to the needs of the persons with life threatening/terminal and/or chronic illnesses.
  • Serve as a member of the IDT and attends meetings
  • Assess patient's spiritual needs within 5 days of admission taking into account mental, emotional, physical, and spiritual stresses.
  • Develop spiritual component of hospice plan of treatment according to patient/family wishes within 14 days and update plan of care biweekly or as needed.
  • Assist IDT members in understanding spiritual factors that influence patient/family coping and influence the hospice plan of care.
  • Visit or contact patients/families as specified in the plan of care.
  • Document services provided according to policy.
  • Provide pastoral care and counseling to include the full range of sacraments and rituals of the religious community in a manner that is meaningful to the patient and family.
  • Develop a program of outreach to the community clergy and religious leaders for the purpose of creating adjunct chaplaincy and represent the Hospice in a positive and professional manner in the community.
  • Serve as a liaison between members of the clergy and hospice patients - for persons of other faith perspectives who wish ministry from within their faith, identify and make referrals to the appropriate clergy or religious leaders in the community.
  • Provide bereavement services as needed or as assigned by the COO including phone calls, visits, and memorial services to family members upon the death of a patient.
  • Review complaints and grievances made by the patient as directed by the COO
  • Participate in community education about Hospice related topics.
  • Plan and participate in Hospice- sponsored memorial services.

Qualifications

A minimum of a four-year college degree from an accredited institution. Must have two years pastoral leadership and/or chaplaincy experience. Experience working with terminally ill patients, their families and caregivers is preferred.
