The Role: We are seeking an experienced Licensed Clinical Social Worker to join our Maternal-Child team. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare providers to address psycho-social barriers and support discharge needs. Your expertise will be invaluable in ensuring seamless transitions from hospital to home and promoting optimal patient outcomes.
Responsibilities:
- Collaborate with RN Case Managers and Utilization Management to develop effective discharge plans that meet patient needs
- Conduct thorough assessments to identify and address psycho-social barriers impacting patient care
- Develop and implement individualized care plans to support patient well-being and recovery
- Communicate effectively with patients, families, and healthcare teams to ensure smooth transitions and optimal care coordination
Requirements: To succeed in this role, you must have a Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school, as well as licensure as a CSW or MSW required by the state of Delaware. Acute Care Social Work experience is highly preferred, particularly in maternal-child settings.
