Job Description
As a licensed clinical social worker, you will play a crucial role in our care management team, working collaboratively with RN case managers and utilization management specialists to ensure seamless patient progression and discharge planning. Your primary responsibility will be to resolve psycho-social barriers and support discharge needs for patients. You will work closely with multidisciplinary teams to address complex patient cases and develop effective treatment plans. Additionally, you will provide emotional support and counseling to patients and their families, helping them navigate the challenges of acute care.
Required Skills and Qualifications
To succeed in this role, you must possess a Master's degree in Social Work (MSW) or a Master of Social Services degree (MSS) from an accredited graduate school. You must also be a licensed clinical social worker (LCSW) or LMSW prior to your start date. Acute care social work experience is preferred, particularly in maternal-child positions that involve working with neonates, children, and families. CCM certification is also beneficial within 18 months of eligibility.
