Description
Responsible for providing basic bedside care to an assigned patient or group of patients under the supervision of a Registered Nurse / Licensed Practical Nurse. Provides close observation of a patient and alerts a nurse on duty of any changes in the patient's condition.
- Performs basic nursing procedures required for all patients such as feeding, turning, transferring, hygiene, taking vital signs, answering call lights, reporting any changes to Registered Nurse, etc.
- Receives assignment and listens to verbal report at start of shift.
- Records patient data in electronic medical record.
- Keeps unit clean and organized by daily cleaning of utility rooms, linen closet, staff lounge, patient rooms, etc.
- Knows, understands, and adheres to departmental policies and hospital policies and procedures applicable to the employee but not limited to personnel, safety, infection control, disaster and dress code.
- Maintains a courteous demeanor by presenting a professional and customer service orientated attitude when interacting with patients, co-workers, families, visitors, physicians, and other health care workers.
- Distributes food trays to patients. Picks up trays and documents portion (%) taken. Returns trays to cafe.
- Assists in transport of patients and belongings upon dismissal. Clears room, removing linens, bedpans, etc.
- Attends Nursing Service meetings and in-service programs on a regular basis.
- Assists co-workers in completion of their work to promote teamwork.
- Assists patients with activities of daily living and allows the patient the opportunity for self-care according to his/her capabilities. Activities of daily living include bathing, hair care, shaving, oral care, face care, skin care,
- Peri Care - to be done daily with the bath or morning care; after every incontinency; for patients with odor problems assuring privacy when performing.
- Nail Care - to be done at least twice a month.
- Intake and nutrition - Monitor and encourage fluid and food intake in accordance with the resident's diet and fluid order. Offer fluids hourly and documents on input/output chart.
- Toileting - Monitor and assist residents with bowel and bladder care.
- P.M. or Hour of Sleep Care - Prepare patients for sleep; offer and administer back rub.
- A.M. Care - To be done on all patients to include oral care, wash face and hands.
- Positioning - All patients are to be correctly positioned when in bed or wheelchair.
- Provide a safe environment for patients as well as yourself and co-workers.
- Assist social services and activity departments as needed.
- Able to competently perform the following skills:
- Vital signs
- Catheter care
- Bowel care
- Collection of specimens.
- Care of urinary equipment (i.e. drainage bags, leg bags, urinals, bedpans, leg bands).
- Use of equipment such (i.e. lift, footboards, heel protectors, walkers, wheelchairs, commodes, etc.)
- Applications of braces, artificial limbs.
- Admission and discharge procedures.
- Intake and output monitoring.
- Post-mortem care.
