Hours: Full-time (5 days/week)

We are seeking independent case management health professionals committed to enhancing access and quality of care for a diverse patient population. Pauquette Center, located in Portage, Wisconsin, is seeking a Case Manager to provide services to consumers with chronic mental health conditions through our CSP. Primary consideration will be given to clinicians with experience in this population and those with a mental health license in the State of Wisconsin. Individuals with a training license or training license eligible will be considered, as will individuals at the bachelor level with career plans to seek a master’s degree in a license eligible field. Emphasizing the need for community needs, we welcome providers adept at engaging with clients from varied backgrounds. We prioritize applicants skilled in effective communication strategies, those eager to contribute to a more inclusive healthcare environment.
The positions are based in the community and require driving to consumers locations; a valid driver's license is required. This is primarily a case management position, with an expectation of providing some therapy using evidence-based practices as part of an overall treatment plan. Successful candidates will be part of the team that includes prescribers, nursing staff, psychologist, and support staff. Ethical documentation is a requirement.

Compensation & Benefits
Compensation is salaried, based on 30 billable hours per week. Fully licensed individuals can expect to earn $60,000 per year. Unlicensed individuals will earn $50,000 per year. Licensed clinicians with 2 years experience or more are eligible for higher compensation.
