Responsibilities RN MDS Coordinator:- Conduct patient assessments to determine care needs.
- Oversee the Minimum Data Set (MDS) process and ensure timely completion.
- Collaborate with the healthcare team to develop and update individualized care plans.
- Ensure compliance with state and federal regulations.
- Maintain accurate patient records and care plans.
- Participate in quality improvement initiatives and audits.
- Work closely with healthcare professionals.
- Provide guidance and education to staff members on MDS and documentation requirements.
Qualifications RN MDS Coordinator:- Current RN license in OH.
- Proven experience as an RN MDS Coordinator in long-term care.
- Knowledge of MDS processes and Medicare/Medicaid regulations.
- Strong organizational and communication skills.
- Detail-oriented with a focus on maintaining accurate records.
- Ability to collaborate effectively within an interdisciplinary team.
