What are the responsibilities and job description for the Physical Therapist position at Pavilion of Logan Square?
Pavilion of Logan Square is seeking a Physical Therapist to join their team! The Physical Therapist plans, develops, organizes, implements, evaluates and directs the physical therapy services, as well as its programs and activities for residents in the skilled care facility.
Physical Therapist Duties and Responsibilities:
- Reviews physicians’ referral, resident’s condition and medical record to determine resident’s qualification for therapy services.
- Administers evaluations, tests or examinations to resident to collect information to determine resident’s needs and therapy required.
- Explains physical therapy policies, procedures and programs to personnel, residents and family members as needed.
- Educates and trains residents and family members concerning prescribed programs and activities.
- Determines appropriate treatment, selecting activities and exercises based on medical and social history of resident.
- Participates in development and implementation of resident assessments, MDS and care plans including initial, annual and quarterly or as needed.
- Assures that a current file is initiated and maintained for each resident being treated including treatment records/minutes, progress notes, treatment plan orders and authorizations.
- Participates in discharge planning, development and implementation of the residents’ discharge plan of care and related assessments.
- All other duties as assigned.
Physical Therapist Qualifications and Skills:
- Ability to read, write, and communicate in English.
- Strong verbal and written communication skills to perform the tasks required.
Physical Therapist Education and Experience:
- Degree in physical therapy from an accredited program.
- Licensed to practice physical therapy in operating state.
- Previous experience in long-term care facility is preferred.
Physical Requirements:
- Must be able to move intermittently throughout the workday.
- Must be able to push, pull move and/or lift a minimum of 25 pounds to a minimum. Height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.
