Job Overview
The Community Associate plays a vital role in providing support and assistance to residents within our community, ensuring their well-being and enhancing their quality of life. This position requires a compassionate individual who is dedicated to fostering a positive environment for residents, particularly those with substance use disorders or requiring specialized support The Community Associate will work closely with other staff members to promote resident rights and facilitate engaging activities that foster to individual needs and life skills.
Duties
- Provide individuals with direct support and coaching to individuals in daily living skills, social interactions and vocational training, while also facilitate connections to community resources and advocating for client needs.
- Assist residents with daily living activities, including personal hygiene, meal preparation, medication reminders and accessing transportation.
- Provide compassionate care to individuals with substance use disorders and other behavioral health issues.
- Assist clients in meeting their needs for food, shelter and clothing.
- Collaborate with staff and social workers to develop clients person centered care plan (PCP).
- Follow up with clients regarding health and service appointments.
- Facilitate social interactions and community engagement activities for residents, enhancing their social well-being.
- Maintain and submit all required records, documents in a timely manner.
- Participate in treatment team meetings.
- Skills
- Experience working in a behavioral health environment is highly valued.
- Familiarity with substance use and recovery support is beneficial.
- Strong interpersonal skills to effectively communicate with clients, families, and team members.
- Basic knowledge of community resources within the Louisville Metro area a plus.
- Background in social work or experience working with people with substance use disorders a plus.
- Excellent organizational skills to manage multiple tasks efficiently while providing attentive care.
