Job Title: Per Diem Nurse (UAS-NY Certified)

Location: Wappingers Falls, New York

Position Overview: We are seeking a skilled and experienced UAS-NY certified nurse to join our team on a per diem basis. The ideal candidate will perform UAS-NY assessments and develop care plans for our 76-bed Assisted Living Program in Wappingers Falls, NY, with the flexibility to support other locations within the Hudson Valley. This role is crucial in ensuring the highest quality of care and compliance with DOH regulations.

Key Responsibilities:
Conduct UAS-NY assessments to evaluate resident needs and develop individualized Plans of Care.
Collaborate with the care team to ensure all staff adhere to established care protocols and standards.
Screen potential residents for eligibility, ensuring they meet the criteria for admission.
Perform other tasks required to ensure a high-quality, DOH-compliant operation.
Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to meet the evolving needs of residents.
Provide training and support to care team members, fostering professional development and a culture of continuous improvement.
Support the 76-bed Assisted Living Program in Wappingers Falls, NY, and have the flexibility to assist other locations within the Hudson Valley as needed.

Qualifications:
Registered Nurse (RN) license in the state of New York.
UAS-NY certification.
Experience in assisted living or a similar healthcare setting.
Strong leadership and team coordination skills.
Excellent communication and interpersonal abilities.
Proficiency in conducting UAS-NY assessments and developing Plans of Care.
