Description
The Psychiatric Nurse Practitioner provides behavioral health care services and medication management to patients in all life cycles. The Psychiatric Nurse Practitioner also participates with a multidisciplinary team to ensure holistic psychosocial and medical needs are met.
Essential Duties and Responsibilities:
• Maintain 36 patient care hours per week at designated site as determined by the Behavioral Health Manager, Chief Population Health Officer, and the Chief Medical Officer. Follow PCC’s policies for scheduling, access, chart documentation and availability for communication regarding clinical matters during regular business hours.
o Patient volume expectation is average 6 patients per session worked. A session is defined as 3 patient care hours plus 1.0 hrs of administration duties. Number of sessions worked is based on FTE status (e.g. 1.0 FTE = 10 sessions).
o The number of sessions per week may be modified based upon clinic need, supervision commitments, etc. Modifications are approved by the Behavioral Health Manager.
• Provide consultation and academic support to residents and attending physicians in the areas of behavioral medicine.
• Participate fully in relevant quality assurance and performance improvement measures.
• Provide mental health consultation and medication management regarding assessment and mental/behavioral health treatment options to established patients.
o Assess patients’ and/or families’ bio-psycho-social situations that result in diagnostic conclusions that include development issues, family dynamics and stressors, and a DSM V diagnosis.
o Document data, assessment, treatment plan and expected outcome in medical record.
• Identify and follow-up on all referrals you have made to assure continuity of care and patient/family needs are met.
• Participate in 1 hour of individual and/or group supervision monthly from a Board Certified Psychiatrist.
• Perform any crisis intervention, individual counseling, family counseling, advocacy that is needed for the patient.
Qualifications
Experience/Training:
• Masters Degree in Nursing from accredited University.
• Board certified/Board eligible child, and/or adolescent and/or adult psychiatrist
• Demonstrated skills in the clinical practice arena and the ability to work and collaborate on a health care team.
• Demonstrated ability to effectively and efficiently handle a demanding workload involving multiple tasks.
• Ability to function in a computer oriented environment possessing intermediate computer skills in word processing, in automated clinical information systems, accessing emails and conducting internet research.
• Ability to communicate effectively in oral and written communications. Bilingual English/Spanish is preferred.
Certifications/Licenses:
• Current and valid driver’s licenses.
• Masters in Nursing from accredited University required.
• Advanced Practice Nursing Licensure in the state of Illinois
• Current CPR Certification
Salary : $121,000 - $139,150
