Description
Lifestyle Coaches are part of a CDC-recognized program at PCC Community Wellness Center, which aims to provide quality healthcare and promote health equity. Coaches guide participants through a year-long program based on the National Diabetes Prevention Program curriculum. The program includes 16 weekly sessions followed by at least 6 monthly follow-up sessions. The goal is to help participants adopt healthier eating habits and increase physical activity. Coaches provide support, share helpful information, and help the group work through challenges. Needed facilitation skills and a passion for helping people make lasting lifestyle changes.
Qualifications
- Lead and facilitate weekly sessions for the first 16-26 weeks, followed by monthly sessions for the rest of the year.
- Create a welcoming, motivating, and non-judgmental group environment.
- Reinforce session material, build on previous discussions, and connect concepts across sessions.
- Encourage group discussions to engage participants and enhance accountability.
- Support participants by providing information, offering encouragement, and helping address any challenges or barriers.
- Guide participants in setting weekly goals.
- Foster shared learning and collaboration within the group.
- Prepare for each session by reviewing lesson plans and giving feedback on participants' food and activity logs.
- Deliver the National Diabetes Prevention Program as designed, using personal coaching strengths and culturally relevant examples without altering core messages.
- Uphold high standards for both self and participants, including consistent attendance, punctuality, weekly weigh-ins, and self-monitoring.
- Be available before and after sessions for questions and follow up on any unanswered queries.
- Contact participants who miss a session to provide support and maintain engagement for both DPP and DSMES program.
- Track and record participant data weekly for submission to the DPP and DSMES.
- Provide ongoing follow-up, obtaining pre-program survey and confidentiality disclosure, and goal setting with patients/families for the DSMES program.
- Work closely with DSMES to help ensure that patients have comprehensive, and diabetes coordinated care plans.
- Distribution of handouts ensures arrangements for veggie box contact and delivery.
- Responsible for providing consistent communication to CDCES and the education team to report patient/family status, ensuring that provided information, and reports clearly describe diabetes progress.
- Tracking clinical lab values, keeping track of attendance list.

- Understanding of behavior change principles and group facilitation.
- Strong communication skills, both one-on-one and in group settings
- Ability to build connections with individuals and foster a sense of community within the group.
- Skill in guiding behavior change without offering specific solutions or advice.
- Experience working with diverse groups of people, including different ages, ethnicities, and backgrounds.
- Excellent active listening skills
- Positive attitude and enthusiasm
- Basic knowledge of health, nutrition, and fitness
- Proficient in MS Office Business Applications to include Outlook, Word, PowerPoint, and Excel and Zoom.
- Physical Demands:
- Must be able to remain in a stationary position 50% of the time.
- Must be able to move around the clinic site 50% of the time.
- Constantly operates a computer, computer printer, copy machine, and telephone.
- Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
- Good organizational skills to handle multiple priorities while remaining professional and calm.
- Strong level of confidentiality due to the sensitivity of materials and information handled.
- Ability to make suggestions on workflow or system efficiency and effectiveness.
- Ability to work independently and be self-directed and flexible.
- Ability to perform functions with minimal supervision.
