Community Health Worker (CHW) Role – Contract Position (1099)
Join our startup social services tech company as a Community Health Worker (CHW) on a contract basis (1099). Caseloads will develop progressively as we grow, offering an opportunity to make a direct impact on addressing housing navigation and other critical needs for underserved communities.
Key Duties:
- Housing Navigation:
- Provide tailored support for unhoused individuals, including identifying shelter options, securing permanent housing, and connecting with housing programs.
- Advocate for clients with landlords, housing authorities, and social service agencies.
- Outreach & Engagement: Identify and engage community members needing health, housing, and social services.
- Field Visits: Conduct in-person assessments, deliver health education, and assist clients in navigating public systems like Medicare/Medicaid.
- Care Coordination: Collaborate with providers to address holistic client needs, including food security, transportation, and healthcare access.
- Crisis Intervention: Address urgent needs for unhoused individuals, including eviction prevention and immediate shelter placement.
- Workshops: Facilitate community sessions focused on housing rights, resources, and public health education.
- Tracking: Maintain accurate records and report outcomes via mobile tools integrated with agency systems.

Job Type: Contract
Pay: $23.51 - $24.75 per hour
