What are the responsibilities and job description for the Direct support Professional Lead position at PCH?
Growing Home Care Agency in need of great team member who is outgoing, a self-starter, determined, dependable, results driven, great team member, great customer service and sale skills.

**Must have experience with direct care and working in the Home Care agency office and Working in the Role of House Manager in A Group Home Setting**
** MUST BE TRUSTWORTHY**
Must have integrity and have the ability to handle pertinent and sensitive information in a HIPPA complaint manner
Must have valid license, must be flexible, and be ready to work.
Must be able to pass fingerprint background check, CPR and First Aid, physical and TB.
Must have knowledge and experience of patient working with the elderly and disabled. Medical experience preferred.

Duties include but are not limited to:
Scheduling of clients and caregivers. Must be able to accurately enter client and caregiver information in Home care software and schedule clients for daily visits.
Must be able to recruit and interview great CNA’s and PCA’s, perform employee on-boarding, orientation, new employee training and termination.
Keep caregiver and clients files updated and all times and in a compliance with state regulations and inspection ready.
Contacting all potential referrals sources to build census.
Arranging nursing visits in accordance with state regulations and assisting in QA audits, ISP and HRST.
Overseeing the Group home daily operations including accurate documentation, caregiver teaching, making doctor's appointments for individuals, accompanying them to the hospital as needed, being on call for emergencies and other duties as assigned.
Must be able to assist with recruiting of new caregivers and schedule caregivers in accordance with agency policy and be able to match caregivers and clients effectively.
Must be able to address client and caregiver concerns in a time sensitive manner and with professionalism at all times and escalate concerns to upper management as needed.
Perform all other duties as assigned by management
