What are the responsibilities and job description for the Case Manager position at PCS Mental Health?
At PCS Mental Health, it's our mission to enable individual persons who have serious mental illness, co-occurring substance use disorder and/or a dual diagnoses of SMI and IDD live a life that is meaningful to her or him in the community on terms she/he defines.
Summary of Job Duties:
This position will develop, implement, and report training/treatment plans for assigned members and participate in crisis management systems, program activities and supervision.
Specific Responsibilities:
- Admission/orientation of new members
- Provide individual training/counseling for assigned members
- Monitor member self-administration of medication
- Complete family contacts for assigned members per level requirement and document within 24 hours of contact
- Complete weekly case management sessions with assigned members and document session within 24 hours of contact
- Ensure emergency contact consents are updated yearly
- Complete Distress Tolerance within 30 days of admission
- Update Distress Tolerance Plan every 6 months
- Facilitate Skills groups and complete documentation within 24 hours of facilitating group
- Complete Advance Directives within 30 days of Admission
- Update Advance Directives annually
- Participate in Weekly Rotation of On Call
- During Assigned week of On-Call be available to go on-site if an emergency arises between the hours of 9 pm and 8 am to support members if needed.
- Complete Incident Reports per incident management processes and policies
- Observe member ALS and UDS; document observations within 24 hours of observation
- Attend Team meetings and staff meetings
- Transport members to appointments, recovery centers, apartments and other destinations per the Community Directors direction
- Attend at least 25 hours of continuing education annually
- Complete trainings as aligned with regulatory compliance
- Knowledgeable of assigned members treatment plan and support with interventions as directed by clinical team
- Ensure that 60% of hours worked are documented in EMR

Qualifications
- High School Diploma with 3 years experience in the Mental Health, Behavioral Health or Drug and Alcohol field or Associates degree with 2 or more years experience in the Mental Health, Behavioral Health or Drug and Alcohol Field or High School Diploma/ Associates degree and 1 year experience at Project Transitoin/ PCS Mental Health.
- Excellent communication, interpersonal, organizational and time management skills.
- Demonstrated ability to work as a part of a multi-disciplinary team.
- High energy individual with strong work ethic and ability to multi-task.
- Must be able to work in a self-directed manner.
- Ability to maintain confidentiality.
