Peace of Mind is seeking a dedicated professional to oversee our Community Services/ Respite department. The Program Coordinator is a full-time position responsible for day-to-day operations regarding individualized home support services. The Program Coordinator will coordinate, communicate, and organize individuals/families needing services and direct support professionals team members. Additional responsibilities will include being available to answer calls and address concerns as they arise which may include, but not limited to scheduling changes, scheduling coverage, and filling in for DSP. Coordinator will work closely with community and vocational support professionals, individuals, and their family as well as case managers.
Position is salaried, minimum of 40 hours per week and on a support by phone when needed. Schedule varies based on individuals needs, must be able to work days, with some evenings, weekends if needed.
Responsibilities:
- Engage individuals and establish trusting collaborative relationships directed towards meeting service needs and support goals/outcomes.
- Ensure person-centered planning and delivery of services.
- Attend and participate in leadership/management meetings.
- Team member development, including new team members and ongoing leadership for all team members.
- Coordinate and conduct team meetings on a regular basis and as needed.
- Participate in the completion of support plans (245D required paperwork)
- Complete staff scheduling and payroll.
- Complete and review individual documentation.
- Complete scorecards for team members, in conjunction with Director.
- Provide on-site mentoring/check-ins and support to individuals or team members.
- Education/Experience
- 2 years of related/relevant experience in the field or in a supervisory position
- Maintains a valid driver’s license and an insured vehicle that meets State of Minnesota’s insurance requirements.
- Pass Minnesota Department of Human Services background check.
- Computer skills/experience specifically with E-mail, Word, and Excel required.
