What are the responsibilities and job description for the Case Manager / Children's Targeted position at Peace River Center?
This is a Bachelor's level position that requires the person to have the ability to prioritize workload and work independently. Assist recipients in gaining access to needed medical, social, educational, and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. A majority of duties will be in the community working with children and families providing services in-home, at daycare, in the school system, and in residential treatment facilities. This position will be based in Bartow and will primarily cover services in Polk County. However, services are available in Highlands and Hardee Counties as needs arise.
Essential Duties/Requirements/Activities:

· Make appropriate contacts with community providers that may be working with the child.
· Maintain accurate records.
· Provide on-site services to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services, i.e., monitoring, linking, advocating, planning and assessing.
· Services will be provided in- home, at daycare, in the school system, and in residential treatment facilities.
· Conduct assessments/interventions which include assessment of substance abuse/co-occurring issues, mental health disorders, and family systems issues.
· Access appropriate PRC and/or Community Resources for the child and family.
· Interact professionally with families, school personnel, inter/intra agency representatives and the general public.
Qualifications:
Education:
A Bachelor's Degree from an accredited university or college
Experience:
With an infield degree at least one-year experience working with children with serious emotional disturbances.
With an out of field degree at least three years' experience working with children with serious emotional disturbances. This may include Internship and Volunteer Services.
Must have knowledge and experience in using local community resources as referrals. Experience in basic child development and family relationships.
Experience with DCF, Child Abuse Laws, and basic mental health knowledge.
Knowledge and Skills:
Excellent verbal communication and writing skills in English
Ability to react to change productively
Ability to make dependent decisions relating to recipients' service needs
Ability to prioritize workload and work independently
Ability to deal with stressful situations or difficult people
Ability to act/react quickly and professionally in a variety of relevant crisis situations
Transportation:
Must have reliable transportation and be of age 21 to pass our DOT requirement
Must have clean driving record - DMV check will be performed upon hire
Must provide and maintain proof of personal vehicle coverage of Bodily Injury Liability
Requires Valid Florida Driver's license

Work Environment:
The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. The noise level in the work environment is usually moderate. Must understand and accept the possibility of exposure to inside environmental conditions such as communicable diseases and physical/verbal abuse from an out of control patient/client.
