Peaceful Pines Senior Living is committed to providing exceptional care and service with compassion. Our community offers a total of 60 independent living units, 29 assisted living units, and 32 memory care units located in the beautiful Black Hills.
The Staff and Resident Care Coordinator reports directly to the Director of Nursing. The Staff and Resident Care Coordinator is responsible for assisting with care coordination for residents at Peaceful Pines Senior Living, managing medication administration efforts, and managing the schedule for the caregiving team. Assists in providing guidance to caregiver staff in accordance with the organization's policies and applicable laws. This position is responsible for supporting efforts directly tied to the direction, coordination, and evaluation of resident care.
DUTIES AND RESPONSIBILITIES TO INCLUDE:
- Ensures the well-being and protection of every resident through the delivery of high-quality care.
- Promotes and supports a positive work environment focused on team building and collaboration.
- Provides on-call backup to community when not on duty.
- Maintains confidentiality of all pertinent personal or health information concerning residents and staff.
- Ensure that all the needs are met for the clients as well as supervises staff that is on the shift.
- Ensures that any incident reports are completed properly and promptly.
- Prepares necessary medical paperwork for all scheduled resident appointments.
- Checks pendant system frequently to ensure proper functioning and timely alert response.
- Assists with compliance in meeting regulations and is available for state licensure surveys.
- Assists with QAPI and safety audits and reporting including the fall tracking spreadsheets.
- Maintains resident files and appropriate documents as required.
- Assists Medication Aides and CNAs in service delivery and training as requested.
- Notify RN of any new doctors' orders that are received and inform RN of any med changes.
- Performs inventory management for medication administration by Medication Aides and checks new medications from the pharmacy.
- Manages inventory or hygiene and incontinence program.
- Communicates to staff any information that staff needs to be aware of.
- Completes any med setups and administration as delegated by the RN.
- Performs Accucheck and insulin administration draws as delegated by the RN.
- Completes changes on MAR per RN's instructions.
- Creates and manages the caregiver schedule.
- Maintains the med carts and med rooms per regulations and audits appropriately.
- Medication changeover.
- Charts appropriately on residents.
- Assists with bathing, dressing, and grooming as needed.
- Audits bathing tasks, updates bath schedules as new residents admit or discharge.
- Manages the oxygen and CPAP equipment for the residents.
- Completes post fall evaluation per policy and under the supervision of the RN.
- Assists with serving meals as needed.
- Assist with picking up shifts on an evenly distributed schedule among nursing staff as needed.
- Actively available for on-call rotation.
- Schedules care conferences and updates care plans in accordance with regulations.
- Other duties as assigned.
GENERAL REQUIREMENTS INCLUDE:
- Employee must be capable of exercising good judgment and ensure that Peaceful Pines Senior Living will provide a safe, comfortable, and secure living environment for the residents.
- Employee must be able to lift, push, and pull 30 lbs of weight without risk of injury.
- Employee must be willing to undergo on the job training before being certified to work with residents.
- Employee must be flexible and willing to adapt to changing situations in a professional manner.
- Employee must be efficient and well organized.
- UMA license required or the ability to obtain it timely.
- Experience in long-term care preferred
- Point Click Care experience preferred.
COMPENSATION/HOURS/BENEFITS:
- Full-time, non-exempt, 40 hours per week generally M-F with some weekends and holidays
- $18.00-$24.00/hour depending on experience and exemption status
