PeaceHealth is seeking a Nurse Practitioner or Physician Assistant to join a vibrant multi-disciplinary Palliative Care team.

Medical, spiritual, and emotional assistance helps patients and their families to ease the symptoms of life-threatening illnesses. Ideal candidates will have a certification and experience in Palliative Care. At PeaceHealth, Our Vision is that every person receives safe, compassionate care; every time, every touch.

Why you’ll love the practice:

• Join a collegial team of 5 providers, with an outpatient census of 80 patients, seeing ~3-7 patients daily.
• 50% home-based care and 50% clinic with the potential of inpatient opportunities, using EPIC EMR.
• Southwest Medical Center and St. John Medical Center have on-site complex case management teams, social workers, chaplains, and partnerships with the Ray Hickey & Richard Nau Hospice Houses serving Clark County and providing coverage in Cowlitz County as needed.
• Schedule: Monday - Friday, ~8:00 am - 5:00 pm, and no after hour call.
