QUALIFIED DEVELOPMENTAL DISABILITY PROFESSIONAL
QUALIFICATIONS: Minimum of Bachelor’s Degree in Behavioral or Social Sciences and at least two years’ experience working with persons with developmental disabilities. Must have ability to work well in a team; excellent interpersonal and writing skills. Must have excellent computer skills including, but not limited to, Microsoft Office, sending and receiving e-mail, accessing web page, entering basic data, use of the Time Keeping System, and common use applications. Valid Indiana Driver’s License and valid auto insurance.
REPORTS TO: Director of Support and Quality Assurance
ESSENTIAL FUNCTIONS: Participate in the admission process regarding all clients on assigned caseload. Ensure that all programmatic needs are met. Coordinate the inter-disciplinary team process and serve as an advocate for all persons on caseload. Monitor the implementation of the Individualized Support Plan. Support the direct program staff in the development of best practices concerning respect/dignity, client choice, functional activities, effective teaching strategies and Active Treatment.
DUTIES:
1. Serve as advocate for all assigned clients; present rights at least annually; ensure all reportable incidents are reported via the state incident reporting process.
2. Advocate for clients to receive all services specified in the ISP; clarify services agreed upon at the end of the ISP meeting; upon receipt of ISP/ NOA, review for accuracy; facilitate any NOA/ ISP changes required with Case Manager; analyze utilization of services at least on a monthly basis; notify program staff of over or under utilization of any client budget; work with program staff to better serve hours on client NOAs.
3. Monitor day and residential services for compliance with IAC-460, CARF, ICFMR, and other program and fiscal standards as applicable.
4. Serve as Intake resource for new admissions. Obtain information regarding referrals, determine appropriateness for various programs, share information with admission team as needed and complete all necessary enrollment forms.
5. Serve as a resource to the team: Monitor the implementation of training programs, facilitate best practice teaching methodologies; facilitate and document staff training as required; facilitate crisis intervention when needed; complete functional assessments/ develop Behavior Support Plans, as needed; attend Team Meetings to ensure best coordination of staff/ activities.
6. Establish master file for all assigned clients; obtain/ complete all required documents; maintain files with periodic review to ensure accurate and complete documentation; document contacts/ events in files. Maintain discontinued files for at least seven years.
7. Develop client goals and objectives for clients; train staff on goals; review, monitor and update goal progress on an on-going basis.
8. Coordinate/ monitor ongoing delivery of Active Treatment to assigned clients with all associated program staff.
9. Schedule and ensure timely meeting notification, chair Initial/Annual Individualized Support Plan meetings, ensure initial plan conference is held within 30 program days of placement.
10. Serve as liaison between clients and community agencies: assist with referrals to appropriate community resources; work cooperatively with other provider agencies that co-serve clients in order to support each client.
11. Coordinate with BDDS and Case Management personnel, providing input as needed.
12. Receive all required trainings in a timely manner and at least twenty-four hours of in-service training annually.
13. Maintain conformance with CARF and all applicable standards, rules, regulations, laws and administrative codes.
14. Assists in completing objectives of the annual Plans as required (i.e. Monthly surveys, meeting attendance, etc.)
15. Attend staff meetings as requested.
16. Assume other duties and responsibilities as assigned.
Required Knowledge, Skills and Ability: Must: 1) be able to lift up to 70 lbs., walk, bend, crouch, kneel, twist, push, pull, squat, turn in the knees, back, wrists and shoulders and grasp with both hand (in order to properly perform the techniques taught in CPR/First Aid, Client Intervention Techniques and Lifting and Carrying Techniques). 2) Must have normal hearing, with the aid of corrective devices, as needed (i.e., hearing aid). Must have normal vision, with the aid of corrective devices, as needed (i.e., glasses or contacts). 3) Must be free from fainting spells, blackouts, dizziness, or seizures. 4)Must have the ability to sit in a normal seated position for extended periods. 5)Must have the ability to endure extended periods of driving. 6) Must have the ability to tolerate exposure to dust, gas, and/or fumes. 7) Must have the ability to exert up to 20-50 pounds of force, occasionally, and/or 10-26 pounds of force frequently. 8) Must be able to speak, read and write in English. 9) Must be able to complete the Peak Community Services training programs and successfully pass associated test to meet competency requirements. 10) Some positions require that employees must be able to pass a TB test.
