What are the responsibilities and job description for the Wellness Coordinator/RN position at Peak Community Services?
Requirements: Valid Registered Nursing License for the State of Indiana
Reports to: Chief Operating Officer
Duties and Responsibilities:
Provide nursing services for ICF / DDE Group Homes licensed by the State of Indiana.
1. Regular Nursing Services will consist of completing Nurse Quarterly Reviews in each group home in Logansport, including:
a. Reviewing medical / health logs
b. Documenting current status
c. Supplementing the Nurse Quarterly Reviews
d. Informing staff of any physician referrals that may arise from Nursing Monthly Reviews
2. Nursing Services may include:
a. Emergency medical consultation, by phone or in person
b. Attendance at new admission meetings
c. Attendance at Individual Support Plan meetings
d. Follow-up on hospital inpatient admissions or emergency room visits / discharges
e. Provide training to staff on medical issues
3. Conduct Wellness Coordinator Services for clients in Logansport consisting of developing, maintaining and monitoring participants involved in the Wellness Plan. Services include:
a. Completing the State’s Risk Assessment Tool
b. Developing, overseeing and maintaining a Wellness Coordinator plan and Risk Plans.
c. Consulting with the individual’s health care providers
d. Consulting with the individual team
e. Being actively involved in team meetings
4. Med Core Training
a. Train required staff using the state approved curriculum
5. Other duties as assigned.
No Nights, Weekends, Holidays, or 12( ) hr. shifts.
