Join our friendly and dedicated team at Pearl Health Center, a family-owned primary and alternative care clinic. We are seeking a licensed Behavioral Health Clinician (LCSW, LMFT, or LPC) to join our team in providing short-term, targeted management of physical health diagnoses. This full-time, in-person position offers a competitive hourly rate of $30-35 per hour, depending on experience. As part of the role, you will be responsible for tracking and analyzing health metrics to ensure compliance with Oregon Health Plan goals, and developing strategies to help our clinic meet these performance targets.

You will work closely with patients to address behavioral and emotional factors impacting their physical health, utilizing evidence-based interventions to achieve improved patient outcomes. Additionally, you will collaborate with a multidisciplinary team to align treatment plans with health metrics, track progress, and implement quality improvement initiatives.

The ideal candidate will be a licensed clinician with experience in both behavioral health treatment and data-driven program management. This is a 40-hour per week role with an in-person presence required.

This role can be negotiated to a part-time position; however, employees working fewer than 32 hours per week will be ineligible for the benefits package offered to full-time staff.

Qualifications:

We are looking for a licensed mental health professional with one of the following credentials:

  • Licensed Clinical Social Worker (LCSW)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Professional Counselor (LPC)

Licensed Associate Clinicians (e.g., CSWA, AMFT, LPC-Associate) are also welcome to apply. However, please note that associate clinicians must be supervised by a licensed clinician. It is the responsibility of the associate clinician to arrange and secure this supervision independently.

Knowledge, Skills and Abilities:

Knowledge of Behavioral Health and Mental Health Treatment:

  • In-depth knowledge of behavioral health practices, including the treatment of mental and emotional disorders related to physical health conditions.
  • Understanding of various therapeutic modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and brief interventions.
  • Familiarity with evidence-based practices in short-term behavioral health management.

Clinical Expertise and Licensure:

  • Current, valid licensure as an LCSW, LMFT, or LPC, with expertise in conducting assessments, developing treatment plans, and providing individual or group therapy.
  • Knowledge of ethical and legal standards in the provision of clinical services.
  • Familiarity with state and federal regulations regarding mental health services and patient confidentiality (e.g., HIPAA).

Technical Proficiency:

  • Strong proficiency in Microsoft Office Suite (Word, Excel, Outlook) and Google Suite (Docs, Sheets, Drive), with the ability to use these tools efficiently for documentation, reporting, and communication.

Patient-Centered Care and Case Management:

  • Ability to develop and implement individualized treatment plans focused on the integration of behavioral health and physical health care needs.
  • Experience in coordinating care with multidisciplinary teams, including physicians, nurses, and other healthcare providers.
  • Capacity to assess, monitor, and track patient progress, ensuring that goals are met within a short-term, focused treatment framework.

Health Metrics Tracking and Compliance:

  • Strong understanding of the Oregon Health Plan's health metrics, quality measures, and performance indicators related to behavioral health services.
  • Experience in tracking, analyzing, and reporting health data to ensure clinic compliance with state or federal health plan goals.
  • Ability to develop, implement, and adjust strategies for meeting Oregon Health Plan targets and improving clinic performance.
  • Knowledge of outcome measures and tools to assess the effectiveness of interventions and care strategies in relation to health goals.

Patient Engagement:

  • Proactively contact patients to schedule necessary health services, providing guidance and support to navigate the healthcare system, ensuring access to appropriate care, and helping patients meet required health metrics for improved outcomes.

Strategic Planning and Program Development:

  • Experience in developing and implementing clinic-wide strategies to meet behavioral health-related health metrics, goals, and performance standards.
  • Ability to work collaboratively with leadership and multidisciplinary teams to create targeted initiatives aimed at improving clinic outcomes and patient care.
  • Skilled in identifying gaps in care, creating process improvements, and designing interventions to enhance clinic performance.

Communication and Interpersonal Skills:

  • Excellent verbal and written communication skills to effectively engage with patients, families, and healthcare team members.
  • Ability to build rapport with patients, demonstrating empathy, sensitivity, and cultural competence.
  • Strong active listening skills to accurately assess patient concerns and provide meaningful support.

Crisis Intervention and Problem-Solving:

  • Ability to assess and manage crisis situations, including suicidal ideation, self-harm, or severe behavioral disturbances.
  • Strong problem-solving skills to address complex behavioral health challenges and create effective interventions.
  • Ability to provide de-escalation strategies and manage conflict in a clinical setting.

Cultural Competence and Sensitivity:

  • Understanding of diverse cultural, socio-economic, and demographic backgrounds, and the ability to tailor interventions to meet the needs of varied populations.
  • Commitment to providing equitable and inclusive care to all patients.

Documentation and Administrative Skills:

  • Strong organizational skills with attention to detail for accurate and timely documentation of assessments, treatment plans, and patient progress notes.
  • Proficient in the use of electronic health records (EHR) and other clinical documentation systems.
  • Ability to manage a caseload efficiently, ensuring all required reports, treatment updates, and communications are completed in a timely manner.

Team Collaboration and Professional Development:

  • Ability to collaborate effectively with other clinicians, healthcare providers, and support staff to achieve comprehensive patient care.
  • Commitment to continuous professional development, staying updated with advances in behavioral health practices and integrating new techniques into clinical practice.
  • Willingness to participate in supervision, peer review, and case consultation.
