What are the responsibilities and job description for the Medical Home Care Coordinator Nurse position at Pediatric & Adolescent Medicine?
Medical Home Care Coordinator Responsibilities:
- Work alongside pediatric primary care providers and practice staff to identify patients with complex needs
- Provide support to high-risk patients including management of multiple conditions to reduce avoidable admission/readmission to a hospital setting
- Build and maintain relationships with community-based agencies and resources to facilitate access to necessary services
- Participate in monthly meetings with PPOC and practice providers
- Coordinate services and follow-up for patients/families/caregivers with social barriers to care (PT1 transportation, non-medical community resources)
- Assist patients/families/caregivers with navigating the healthcare system and promote patient/family/caregivers' self-care and advocacy
- Obtain necessary medical equipment, medications, and supplies (prior authorizations, FMLA forms, adaptive equipment)
- Promote clear communication among patients/families/caregivers and the care team through integrated care planning, team meeting and collaboration with community-based agencies.
- Act as an educational liaison support.
- Coordinate appropriate follow-up for patients and families following a transition of care, such as a hospital admission/discharge, emergency room visit, or specialist visit
- Parent/Patient troubleshooting (for care coordination issues)
- Referrals related to developmental-behavioral medicine, neuropsychology, and psychiatry
- Triage phone and clinical floor coverage when needed
Requirements:
- Nursing degree (RN or LPN)
- Experience navigating community health
- Experience partnering with families and agency services to coordinate care
- Experience working with both pediatric and adult families and services
- Ability to work within a team to collaborate regarding care
- Ability to use critical thinking skills to problem solve independently
- Must possess strong communication skills

Pay: $30.00 - $35.00 per hour
