This role collaborates with Care Management teams, primary care providers, and payor partners to improve asthma self-management, medication adherence, specialty follow-up, and symptom and trigger awareness. The goal is to increase the quality of care, enhance patient self-management skills, and reduce preventable ER visits and hospitalizations through structured education and care coordination.

EDUCATION:
Associate’s degree or higher in Nursing, Respiratory Therapy, Pharmacy, Public Health, Health Education, or Community Health, Medical Assistant or Social Work background. Bachelor’s Degree preferred.

Patient & Family Education
- Conduct virtual asthma education sessions for pediatric patients and their caregivers.
- Teach proper inhaler and spacer techniques, symptom monitoring, and trigger avoidance strategies.
- Reinforce medication adherence and the importance of controller versus rescue medications.
- Provide self-management strategies to reduce exacerbations and improve asthma control.
- Partner with Care Management teams and primary care providers to ensure coordinated asthma care.
- Educate patients on the importance of routine specialty follow-ups with pulmonologists and allergists.
- Identify high-risk patients with uncontrolled asthma and escalate for intervention.
- Support asthma quality measures such as medication compliance, follow-up visits, and self-management outcomes.
- Collaborate with payor partners and quality teams to align on asthma care initiatives and track performance improvements.
- Assist in implementing standardized clinical pathways for asthma management across practices.
- Provide training to clinicians, medical assistants, and care coordinators on asthma management best practices.
- Collaborate with schools, community health organizations, and PPECs to reinforce asthma education beyond clinic visits.
- Develop and share patient-friendly educational materials and outreach efforts.
- Completes documentation in the care management platform and electronic health record.
EDUCATION:
Associate’s degree or higher in Nursing, Respiratory Therapy, Pharmacy, Public Health, Health Education, or Community Health, Medical Assistant or Social Work background. Bachelor’s Degree preferred.
Experience
- Minimum of 2 years of experience in asthma education, respiratory therapy, or care coordination.
- Experience in telehealth/virtual patient education preferred.
- Candidates must have at least 1,000 hours of experience in providing direct asthma education, management, or counseling to patients within the last five years.
- Certified Asthma Educator (AE-C) required.
- Strong understanding of asthma pathophysiology, treatment guidelines, and medication protocols.
- Excellent communication and interpersonal skills to engage with clinical teams, patients, and caregivers.
- Strong organizational and time management skills to coordinate patient outreach and education.
- Proficiency in electronic health records (EHR) and documentation.
- Knowledge of medical billing and health records maintenance.
- Knowledge of laws governing the protection of patients’ private health information.
