Monarch Maples Pediatrics in St. Albans is seeking a Community Health Worker who is a proactive individual dedicated to making a positive impact on the community's health and well-being.

JOB SUMMARY

The Community Health Worker will provide comprehensive support services to patients and their families with medical, dental, social, and mental health needs. The position includes interacting with patients in a Pediatric Primary Care setting, in the patient’s home, and in the community. The Community Health Worker will be responsible for establishing interpersonal relationships with patients and their families. The individual will function in a multidisciplinary team providing holistic care aimed at improving overall health. The Community Health Worker will be responsible for helping patients and their families navigate and access community services and other resources. The Community Health Worker will aim to assist the patient and or family in their ability to adhere to health recommendations, understand and make decisions that will reduce inappropriate healthcare utilization and enhance communication between the patient, family, and the health care team.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Interacts with patients, community members, families, community-based resources, and the members of the care team to provide a range of care services. Specific duties and responsibilities include:

  • Care Coordination
  • Providing health education and coaching in the patients preferred format.
  • Facilitating health promotion and education
  • Assisting patients in accessing their medical homes.
  • Organizing and providing patient outreach
  • Promoting patient health engagement and education
  • Assessing and responding to an individual’s needs involving Social Determinants of Health including but not limited to transportation, nutrition, housing, etc.
  • Helping patients develop health management plans and goals.
  • Helping patients understand their care plan and develop plans to achieve it.
  • Helping patients to utilize community resources.
  • Continuously assessing a patient’s eligibility for State, Federal and Community-based programs designed to improve and meet a patient’s Social Determinants of Health needs.
  • Follow up on referrals to specialist and community-based organizations
  • Assisting patients in obtaining affordable medications.
  • Supporting behavioral change using motivational interviewing.
  • Advocating for individual and community needs.
  • Performing other functions or duties as assigned.

  • Strong computer skills. (Electronic Medical Record, Microsoft Office- Word, Excel)

EDUCATION

High school education or equivalent required

Associates degree in a related health field is a plus.

EXPERIENCE

Personal experience with utilizing services to support your need for Food, Housing, Transportation, Financial, and Safety, etc. is a plus.

Prior experience working with community-based programs preferred.

Prior experience in a healthcare and or health related field is a plus.

Job Type: Full-time

Pay: $20.00 - $28.00 per hour

Expected hours: 40 per week
