Experienced registered nurse providing professional telephone triage services and health advice to patients. Conducts thorough assessments of patient symptoms and offers appropriate medical guidance, including home care instructions or recommendations for further evaluation. Adheres to established protocols, collaborates with healthcare providers, and ensures accurate documentation of all patient interactions.
ESSENTIAL FUNCTIONS
- Conducts telephone assessments to determine the severity of symptoms and provides appropriate medical guidance.
- Directs patients to emergency care, urgent care, or primary care providers based on clinical assessment and established protocols.
- Performs brief evaluations of patient conditions, including chief complaints, vital signs (when available), and mental status.
- Utilizes clinical decision support tools and algorithms to guide triage decisions, consulting physicians as necessary.
- Provides immediate referral to emergency services for high-risk symptoms, such as chest pain, abdominal pain, or severe headaches.
- Offers home care advice for non-emergent conditions and facilitates appropriate follow-up care.
- Schedules appointments for patients who require non-urgent medical attention.
- Responds to patient inquiries in an "Ask a Nurse" capacity, addressing common health concerns and preventative care questions.
- Reviews overnight and weekend answering service logs, following up with patients requiring additional care or scheduling.
- Contacts patients for follow-up on lab results per provider instructions.
- Prescribes medications based on standing orders when authorized.
- Assists with pharmacy-related inquiries and medication issues throughout the day.
- Supports clinic operations by assisting with patient flow, including rooming patients, administering injections, or performing lab work as needed.

Required Skills & Abilities

- Skilled in rapid, evidence-based decision-making during emergency situations.
- Excellent communication skills with the ability to provide clear, compassionate guidance to patients and caregivers.
- Strong critical thinking and problem-solving abilities.
- Ability to collaborate effectively with physicians, nurses, and healthcare staff.
- Registered Nurse (RN) degree required; BSN or MSN preferred.
- Minimum of five (5) years of RN experience, with at least one to two (1-2) years in telephone triage, preferably in a clinical or medical practice setting.
- Current and unrestricted state RN license.
- Valid CPR certification.
