Case Manager
Overview: Pee Dee Healthy Start (PDHS) is seeking a case manager to provide outreach and support activities to juveniles fourteen (14) years-old to seventeen (17) years-old and their families to prevent, deter, and reduce juvenile delinquency in five (5) targeted counties. The Case Manager is a participant/client facing team member critical to the PDHS Bridging the Gap team who works directly with juveniles and their families, law enforcement, and the community has a unique understanding with culture competence as a trusted member of the community in which they serve. The Case Manager will facilitate individual, group, family, and community activities focusing on juvenile delinquency education and prevention. The Case Manager will also work as part of an interdisciplinary team to assess youth participants’ needs and connect the juveniles with a wide range of necessary resources.
This position is federally funded.
Summary:
Salary: $40,000-$48,000

Education: Bachelor’s degree in social science, education, social work, or related field (preferred). Associate’s degree, high school diploma/equivalent required. Relevant experience working with juveniles, young adults, and justice-involved will be considered. Must be able to communicate effectively both orally and in writing.
Duties and Responsibilities: Conducts outreach and recruitment services to identify and enroll juveniles and their families in targeted counties. Outreach services include passing out flyers or other relevant materials by community canvassing (documenting who you spoke with), attendance at community events (in-person and virtual), establishing/ maintaining relationships with community/social service organizations, schools, barbershops, hair salons, apartment complexes, housing authorities, shelters, etc. local colleges, etc. Social media platforms can be used to recruit and follow up with PDHS participants. The Case Manager will coordinate and facilitate activities to prevent, deter, and reduce juvenile delinquency. This position is responsible for the intake process and complete care coordination of needs of assigned participants that can include referrals for services such as mental health, medical, social support, developmental, behavioral health, educational, etc.
Required Knowledge and Skills: Must understand the core services and philosophy of PDHS, Inc. and the populations served. Help participants and their families meet their needs to prevent and reduce juvenile delinquency. Provide requisite referrals to connect them to programming and needed community resources by strong follow-up and follow-through mindset. Must have good customer service skills, reliable transportation, current driver’s license and provide timely documentation of services.
