Summary/Objective: The Clinical Manager oversees the day-to-day clinical operations of the home care and hospice programs and collaborates with the leadership team to ensure ongoing superior care for patients and families.
Accountability:
The Clinical Manager reports to the Executive Director.
Essential Functions:

1. Performs annual performance reviews and field supervision assessments for all field staff
2. Coordinates vacation time off and coverage
3. Over sees clinical orientation for all field staff with the assistance of HR team
4. Collaborate with leadership team to ensure home care and hospice programs meet agency financial goals
5. Collaborates with intake/referral department when patients transition from home care to hospice services
6. Provides oversight of all patient care services and personnel including:
a. Collaborating with scheduling on patient and personnel assignments when needed
b. Coordinating patient care
c. Advising on patient referrals when needed
d. Assuring that patient needs are continually assessed
e. Assuring the development, implementation, and updates of individualized plans of care
7. Responsible for the day-to-day management and coordination of field staff
8. Demonstrates ability to manage tasks accurately and efficiently
9. Assists with the quality management of the home care program under the direction of the Quality Director
10. Collaborate with the Quality Director for utilization review of home health patients for all services
11. Maintains and enhances the current level of knowledge relative to professional practice, as well as continuing education requirements necessary for licensure and certification
12. Renders patient care with a high degree of service excellence
a. Responsible for responding to all patient complaints by following Opportunity to Improve protocol and including the Quality Director
13. Consistently meets & promotes productivity guidelines, timely documentation requirements, and agency expectations for clinical performance
14. Help with skilled visits as needed and participates in the RN on-call rotation when necessary
15. Participates in being a resource/backup to the on-call nurse as applicable
16. Consistently meets and promotes the following:
a. Performing all assessments, treatments, and documentation per Agency policy and standards which include State and Federal regulations.
b. Developing, implementing, evaluating and modifying clinical services to patients under a physician’s plan of treatment.
c. Utilizing an Evidenced-Based Chronic Illness Management approach to improve home care patient outcomes.
d. Documenting interdisciplinary communication.
e. Instructing patients and family members regarding the plan of care resulting in the patient/family assuming increased responsibility for care when appropriate.
17. Assists in referring patients to other community resources as needed
18. Responsible for ensuring the coordination of home care evaluations and admissions
19. Works with clinical staff for consistency in treatments and documentation
20. Works with the Quality Director to establish and track key program indicators to meet or exceed national averages
21. Serves as a positive role model for staff
22. Provides on-going coaching for staff to ensure compliance with accepted standards of practice; proactively takes action to address deficiencies in practice in a timely manner
23. Enters discipline orders in EMR when needed
24. Calls PCP for verbal authorization at SOC, ROC, Recerts when needed
25. Discontinues discipline orders upon Transfer/Discharge when needed
26. Assists Implementation Assistant in entering medications in EMR when needed
27. Monitors weekly clinical schedule for consistency
28. Facilitates interdisciplinary group (IDG) meetings
29. Assists with triage calls when needed
30. Sets up training for new field staff and monitors progress to goals
31. Assists Finance Assistant in reconciling timecards with EMR documented time
32. Monitors timecards and manages judicious use of Overtime when necessary
33. Approves timecards in a timely manner
34. Continually assesses staffing needs
35. Coordinates with Business Development Liaison & HR Generalist to ensure job advertisements are accurate and appropriate and conduct interviews
36. Monitors dissemination of medical supplies used for patients and ensure supplies are being entered into the EMR.
37. Maintains and disseminates field staff supplies when new supplies or replacements are needed
38. Facilitates meetings for field staff
39. Provides Matrix Care training to new and current employees
40. Assists with all other duties as assigned
Expectations:
The Clinical Manager position at Pemi-Baker Hospice and Home Health (PBH&HH) is a full-time, exempt role requiring a commitment of at least 80 hours per pay period (Bi-Weekly), with additional hours expected as necessary to meet organizational and patient care needs. The role involves overseeing clinical staff, social worker, and chaplain for home care and hospice programs, ensuring high-quality patient care, and maintaining compliance with regulatory standards.

Supervisory Responsibility:
Direct supervision of members of the clinical staff, social worker, and chaplain
Work Environment:
The job operates in a clerical office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This job is clinical as well as administrative and does perform patient visits in the field when needed. This role is eligible for remote work with manager’s approval.
Physical Demands:
This is largely a sedentary role; however, some in-home visits are required. Work might also require the ability to lift files, open filing cabinets and bend or stand as necessary.
Travel:
Travel is expected for this position.
Qualifications:
1. Associate’s degree in nursing required, bachelor’s degree preferred
2. Previous management experience preferred
3. New Hampshire RN license in good standing
4. Must be able to read, write and speak English fluently
5. Must be available to work Saturday and Sunday if needed
6. Must be able to supply a valid driver’s license without restrictions
7. Must be able to sit for long periods of time viewing small print
8. Must comply with PBH&HH safety and dress code policies
9. Must be able to meet the lifting requirements of the position
10. Must pass a criminal background, driving record, and Bureau of Elderly and Adult Services check
11. Must be able to provide eligibility to work in United States paperwork within 3 days of hire.
12. Must have reliable transportation and be able to supply proof of car insurance in the amount of $100,000/$300,000 coverage.
