Responsibilities:
- Assess individual’s needs and provide nursing care as indicated
- Monitors individuals’ health progress and provides required documentation
- Provide direct and delegated care, including clinical assessments, med administration, and hands-on nursing
- Coordinate medical appointments, maintain health tracking, and develop personalized care plans
- Support and educate care staff and LPNs, ensuring best practices are followed
- Visit individuals’ homes weekly, keeping accurate documentation and ensuring continuity of care
- Be a key member of a collaborative, interdisciplinary team focused on whole-person health
Requirements
- Active NYS RN License
- Experience or passion for working with individuals with developmental disabilities
- Strong communication, time management, and critical thinking skills
- CPR & Infection Control certification (or willingness to update)
- Flexibility for rotating shifts and reliable coverage throughout the year
- Travel and Holidays required
