Job Descriptions : Plans, implements, administers, and monitors pharmacy services to provide the highest quality patient care and customer service in accordance with the philosophy, goals and objectives of Pender Community Hospital / Medical Clinics and The Apothecary Shop.Performs day-to-day operations, which includes problem solving issues and ensuring effective processing, works with other departments in providing services and resources, monitors workflow and departmental processes and supervises technician departmental functions.Performs day-to-day operations of The Apothecary shop, along with managerial duties, effectively and in the absence of the Pharmacy Manager.Establishes controls to promote confidentiality of records and adhere to pertinent laws and regulations (regulatory), coordinates policies for release of information, analyzes records to insure completeness and accuracy, and determines methods for obtaining complete and accurate records.Reads, extracts and interprets information in patient charts accurately and in a timely manner, provides clinical consultation and clarification to practitioners. Suggests appropriate cost-effective therapeutic alternatives to medical staff as needed.Provides drug information to patients and their families, educates clinical staff in regard to standards of practice and disease management guidelines and provides medication education and counseling to patients as needed.Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications, detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner, provides accurate, adequate and timely drug information to the professional staff and promotes rational drug therapy selection per best practice recommendations.Participates in the quality improvement and medication use review activities of the department. Collects data, conducts quality monitors and inspections, and maintains logs, records and other documentation as assigned, stores and preserves vaccines, serums and other drugs subject to deterioration. Ensures that any outdated or unusable drugs are removed and quarantined until they are removed from the facility by a reverse distributor.Monitors the use of the electronic health record to ensure accuracy of information in patient chartsUpdates and monitors new NDCs & barcodes for formulary accuracy, reviews daily charges for accuracy and coordinates with pharmacy technician for charge discrepancies & follow up actions and initiates medication reconciliation activities in collaboration with the patient's primary care provider.Recommends, adjusts, and documents medication changes in collaboration with the patient's primary care provider, orders appropriate laboratory tests to monitor medication therapy in collaboration with the patient's primary care provider, follows facility-approved pharmacy driven protocols and coordinates care with the provider for additional interventions and participates in Quality Assurance programs and ideas.Performs 340B support to the facility as requested (examples : monthly audits, EHR accuracy for medication dispensing, split-billing software review for purchasing, etc). collaborates on development of Medication Safety Committee projects and meeting topics and reviews departmental policies as requested and provides feedback for revisionsReviews order sets for best practices and offers feedback for revisionsOther clinical activities to enhance patient care and medication safety as requestedAttends education and training programs. Reviews literature and other materials pertinent to clinical pharmacist roleCompletes minimum of 30 continuing education activities each license renewalPerforms other duties as assigned.Hours : Minimum 36 hours / week, Monday-Friday, 8 : 00am-5 : 30pm, rotating Saturdays 8 : 00am-12 : 00pmRequired Experience : QUALIFICATIONSEducation, Experience : Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD degreeCertificates, Licensure, Registrations : Active Pharmacist License in the State of NebraskaGENERAL REQUIREMENTSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
