Some of the Job Duties:

• Prescribe medication, examine patients, diagnose illnesses and provide treatment.
• Order, interpret, and evaluate diagnostic tests.
• Gather health histories, perform physical examinations and diagnostic procedures.
• Providing health promotion, prevention, and treatments to patients.
• Diagnosing and treating acute and chronic conditions such as diabetes, high blood pressure, infections, and injuries.
• Prescribing medications when needed to ensure patient health and well-being.
• Develop and implement treatment plans.
• Dictate and document all patient interactions.
• Notify managing physician about unusual or unexpected events or problems.
• Effectively communicate with diverse populations, including physicians, employees, patients, and families.
• Maintains confidentiality of all patient records.
• Checks in with clients about test results and prognoses.
• Advises patients regarding prescriptions and treatment plans.
• Follows all federal, state, and nursing guidelines, regulations, and standards.
• Orders and interprets diagnostic tests, including x-rays, blood work, and scans.
• Refers patients to specialists if necessary and analyzes medical records and reports.
• Updates patient charts and uploads to Electronic Health Record (EHR) software.
• Works with a team of Medical Assistants and doctors to provide quality patient care.
• Examines patients, including discussing medical history and symptoms and performing physicals.

Requirements:

• Communicates effectively, speaking multiple languages is preferred.
• Nurse Practitioner and Registered Nurse licenses in the state.
• Board certification as a Nurse Practitioner in the specialty is preferred.
• National board certification as a Nurse Practitioner from an accredited agency.
• Master’s in Nursing required, Doctorate of Nursing preferred.
• Two or more years of clinical experience as a nursing practitioner at a physician practice or in a hospital with a related specialty is strongly preferred.
• Collaborates with nurse practitioners, registered nurses, doctors, and other medical professionals.
• Problem-solves well independently and as part of a team.
• Develops strong relationships with patients and their families and has a stellar bedside manner.

Qualifications and Skills:

• Proficient EHR user, preferrably EPIC
• Experience in program development, particularly in behavioral health, harm reduction, or underserved populations.
• Familiarity with Electronic Health Records (EHR) systems and their integration into clinical workflows.
• Demonstrated commitment to advancing health equity and addressing disparities in care.
• The essential functions of this position include, but are not limited to: sitting, standing, walking, bending, stooping, pushing, pulling, lifting (up to 25 lbs.), typing, writing (in English), reading (English), speaking (English), seeing, hearing, driving, basic mathematical calculations, problem solving, reasoning, composition, and decision-making.

Pay: Up to $75,000.00 per year
