Social Worker (LMSW) Position Summary
The Social Worker screens patients to evaluate case management needs and utilizes tools to assist patient’s needs. This position develops discharge plans by creating synergy between the level of care needed and resources available. The Social Worker will work with local agencies when needed, such as Adult Protective Services and Child Protective Services, and will help patients with other needed resources, such as rehabilitation facilities and long-term care.
Social Worker (LMSW) Position Requirements
Education
- Graduate of a master's degree program in social work from a college or university approved by the Council of Social Work.
- Licensure as a Social Worker in the state of MD required.
Experience
- At least one year of post graduate work experience preferred.
Social Worker (LMSW) Work Schedule
- Day shift Monday-Friday, Evening and weekend work as assigned.
