Summary :
AS THE SOCIAL SERVICE COORDINATOR, you'll be responsible for assisting patients and families in planning for post hospital care; to perform social assessment for discharge planning; to develop and maintain files on community resources and to provide direct social work services to inpatient units and outpatient services as necessary.
Other information : QUALIFICATIONS :
- Have Bachelor's degree in social work or other related field
- Have one year of Social Work experience
- Have current Pennsylvania driver's license if assigned to Home Health Care
- Private vehicle and active automobile insurance
- Strong organizational skills; critical thinking and problem solving skills
- Must be dependable and self-directed
- Have ability to interact effectively with patients, families and all members of the care team
- Have basic computer skills
