Summary:
Ensures TCM services are provided with attention to assigned caseload as well as in complement to and in cooperation with TCM Program specific requirements for and facility-wide expectation of Team-Oriented service provision.
Submission of Service Documentation forms for billing, regulatory and monitoring purposes occurs as mandated by the TCM program and demonstrate the required productivity of TCM service provision units.
Completes comprehensive Service Planning as mandated by the Office of Mental Health, the Targeted Case Management Unit, Hall-Mercer, the TCM Program and regulatory agencies.
Provides TCM Services as mandated by the Office of Mental Health, the Targeted Case Management Unit, Hall-Mercer, the TCM Program and regulatory agencies.

Summary:
- Provides Targeted Case Management for adults with a serious mental illness. The Targeted Case Manager assists consumers in accessing resources and services that build upon strengths and meet needs in order to enable an individual to function at the highest attainable level in the community.
- Adheres to Compliance standards in service provision and professional development as mandated by the Office of Mental Health, the Targeted Case Management Unit, Hall-Mercer and the TCM Program.

- Education Specialization: Equivalent Experience:
- And 2 years MH/MR Experience
- H.S. Diploma/GED (Required)
