Summary:
- Program Managers are coaches, connectors, and builders in the organization, primarily supporting and inspiring community health workers (CHWs) to do creative, life-changing work with patients. CHWs are people with a natural inclination to help others and who share life circumstances (e.g., race, socioeconomic status, language) with the people they serve. CHWs provide social care, support, and navigation to help patients achieve better health and well-being. Program Managers coach CHWs using weekly supervision time, routine observations of CHWs in action, and quality reports to assess qualitative and quantitative patient outcomes. They connect with clinical partners and other staff throughout the health system to integrate CHWs onto clinical teams and ensure smooth patient enrollment processes and ongoing support. Program Managers build upon the Center's success by leading strategic initiatives that strengthen care outcomes, operations, and organizational culture and wellness.
- Leads a team of up to 6 CHWs and ensures each CHW is achieving patient outcome targets by consistently and accurately producing monthly quality reports of CHWs’ work and coaching CHWs to develop their skills
- Uses Center’s quality tools (Quality Dashboard, Report Generator) and weekly deliverables to assess and address CHW performance. Proactively provides trends and needed next steps to the supervisor during 1:1s
- Integrates CHWs with care teams within our hospitals and primary care clinics. Engages at least quarterly with assigned clinical partners to ensure smooth operations
- Offers ideas and participates in projects to strengthen our program and organizational culture
- Leads an organizational strategy that advances the Center’s mission and supports a professional development area or interest of the Program Manager
- HS Diploma
- 5-7 years of experience in clinical social work, leadership experience with nonprofit or community-based organizations, and service in low-income communities
